IUD Referral Protocol
1. Referring Doctor Office to Fax:
- Referral form;
- Medical summary;
- Medication list.
2. Referral Received:
2.1. Our office will contact patient for appointment booking;
3.1. Notification of appointment date and time will be faxed to referring Doctor’s office.
3. Appointment Booking:
3.1. First visit / consultation / 30 minutes;
3.2. Second visit / procedure/ 45 minutes;
3.3. Third visit / string check (one month later)/ 30 minutes
3.4. Follow up visit / if heavy bleeding (6 months later)/ 30 minutes
Referral Completed:
4.1. Doctor’s consultation report will be faxed to the referring Doctor’s office.
